Your First Visit

At Westside, we strive to provide outstanding customer service and effective treatment to facilitate an early durable return to work or play.

During the initial telephone contact, the administrative staff will provide:

  • the name of the treating therapist
  • the time and day of the appointment
  • the location of the clinic
  • items to bring

You will then be telephoned the day before the appointment as a reminder.

Please arrive 5-10 minutes early to complete paper work. You must sign a consent to treatment and may need to complete other important forms.
